Our Team
Your standing PCP may continue to provide care and work with our hospice team, or you may choose to designate a Hospice MD as your PCP
Our nurses as your case manager will provide hands on skilled care working with you and family for options to manage your illness and provide comfort
Our Hospice aides or C.N.A. will assist with making everyday task (bathing, grooming, dressing) easier while maintaining your dignity and independence
Social Workers support patient and family to assist with practical concerns (financial issues, new living arrangements, wills and burial arrangements)
A chaplain initiates contact with the patient and family to determine needs and provide spiritual support. Provided with individuals choice*
Our Skilled volunteers provide companionship and at times, respite care for caretakers or family who are in need of a short break.
Bereavement Counselors are available to assist loved ones as they grieve Services include individual counseling, support groups, and memorial services
